| 1. Birth certificate information | ||
| 2. Proof of Guardianship (Official court documents or copies of same) | ||
| 3. Complete immunization records | ||
| 4. Proof of residency and legal guardianship for: | ||
| a) If student is over 18 years of age, then the residency proof must be for the student | ||
| b) If student is under 18 years of age, then the parent or legal guardian must provide proof of residency and guardianship | ||
| Examples: Documentation of house or property ownership, rent receipt, electric or telephone bills, proof of legal guardianship | ||
| 5. Completion of registration forms: Sign release of information for educational files and materials from previous educational placement, including any special education records, if applicable. | ||
Students will be scheduled for an entry appointment with appropriate school staff as soon as possible following receipt of information listed above. Hand carried materials are not considered official, but are helpful when preparing a schedule if official records are not available.
Students over age 16 who have previously dropped out of school and seek to re-enroll at Pembroke Academy, may do so only at the beginning of the school year or semester.
