Dear Parent/Guardian:
Attached is a copy of the Pembroke Academy Athletic Packet. Inside you will find the necessary documents to ensure that your son/daughter is eligible to participate in the Pembroke Academy Athletic Program.
Valid Physical Examination forms are required in order to participate in Athletics for the Pembroke Academy. Physicals of incoming grade 9 students in September 2008 should be completed on or after August 1st, 2007. This would apply to any new student-athletes or transfers as well. This physical will remain valid for all sports grades 9 through 12, barring any change in his/her medical condition.
High School students should return their completed packets to their respective coach, or the PA Main Office in the file folder labeled “PA Athletic Forms”, or to the Athletic Office that is located in the Blanchard House.
No student-athlete will be allowed to try-out/practice without being medically and academically cleared first.
Seasonal try-out / practice dates, times and locations will be posted on the PA school website under Athletics, and in local newspapers following pre-season coaches meetings.
As the Athletic Director for Pembroke Academy, I would ask that you assist me in ensuring that your son/daughter have the necessary documentation to participate in our Athletic Program. Please contact me if you have any questions.
